The question was how it was done... not what it looked like.
Unfortunately, I have no input because I haven't done the show. The obvious answer seems to be to just have Jack or someone knock her over in the heat of the argument.

Well, how was it done?
It looks like there is a peg in the stage that goes into a small hole in Milky's leg. Someone under the stage just pushed up on the peg. I think...?

Yes, but not all stages have peg holes in them. Just like not all stages have wonderfully convenient magical moving belts on the front of the stage to utilize as the OBC did. So where does that leave the lower budget, solid staged shows when they're trying to copy the brilliant original death?
I think discreetly tipping over the cow in the middle of the argument is an excellent idea. I haven't been in the show yet (next year probably tho!) so i can't specify any examples.

The one I saw had a person in a really funny cow suit and she played the part really well. It was so funny. She even got the last bow, after the Witch.
